TMV Prep

Open patient’s chart and verify that the patient is diabetic. Otherwise write “NO-PRE-TMV CALL NEEDED” in the appointment comments. (Similar to part 6.)

Check for CGMs and Pumps CGMs (Libre, Dexcom etc.)

Pumps (Tandem, Medtronic, Omnipods etc.)

Morning TMV Duties
-
Filter to see all TMV appointments for the day
-
Click through each patient to find the patients with Diabetic CGMs and pumps
-
These will be ​most readily seen as a Flag (located in the banner at the top of the patient's chart)

Open the appropriate website according to the Flag on the diabetic patient's chart​
-
Check to see if they are linked or if there is recent (within 2 days) blood glucose data available, and print for the provider (specific guidelines are listed at the download computers)

If there is not, call and/or send the patient a Patient Portal Message with instructions for their device which are listed in the below section

If they're able to link or upload then print the device report (specific guidelines are listed at the download computers)

If not, then check once again 1 hour before their TMV appointment​

Once device reports are printed, place them horizontally in the appropriate fee slips on each providers desk​

If there are issues with patients being unable to upload (such as they are not with their download cord, or they are using a meter), write a note for the provider explaining this and place it in the fee slip​
